Academic Fresh Start Application

Instructions

  1. Download the Academic Fresh Start Application. (For full form functionality—including electronic signature functionality—please download the form to your computer and open in either Adobe Pro or Reader.   2. Fill it out.
  3. Save a copy to your computer.
  4. Upload and submit the application to the appropriate college using the Submission Form below.

The below criteria must be satisfied:

  • Script Approval affirmation must be checked AND
  • Affirm Accessibility must be checked AND
  • You have an iFrame in the script without a title, use the 'iFrame Title' input to add one.

The script will not display on the live site until all issues above are resolved.
